How to Report an Incident:
Reporting an incident is as easy as picking up the phone and dialing 866-943-5787. Your call will be answered by a trained specialist who will guide you through the details of your concern. You will then be provided with a case number. Be sure to write this number down and keep it in a safe and secure place. Your interview will not be recorded. The interviewer will type notes of the conversation, and you can be assured all information provided is completely confidential and anonymous. Within one business-day of your call, a summary of the interview will be forwarded to this institution. You will then be able to follow-up on the status of your concern by using the case number you were given.
